In order to correct the information relating to incorrect dates, place of birth and/or identity number a duplicate may be requested by fulfilling the following requirements:
Original identity card (element to be corrected).
Two (2) full-face photographs in color with white or blue background, size 2.5X3.0 cm.
Original birth certificate with the corresponding notation.
Rh. factor and blood type.
