Card and has lost it or has been stolen, must apply for its duplicate at a Colombian Consulate.
Requirements:
- Appear in person.
- Number of the I.D. Card.
- Show document identifying the individual as Colombian.
- Present an original document of the Police Report or fill the relevant
- confirmation of the fact.
- Three (3) frontal photographs in color with white or blue background and dark
- clothing, size 4X5 cm. If bald or with very light or white hair, the background
- must be blue.
- Blood certificate with type and RH factor.
- Voucher for deposit of the sum paid for the duplicate of the document.
Fee: USD32.81
